WELCOME

We are pleased to welcome you to the office of Drs.
Trent Quinlan, Patrick Farrell, and David Denman. We are board certified in the specialty of Otolaryngology, Head,
and Neck surgery. Our medical practice is limited to adult
and pediatric disorders of the ear, nose, sinuses, larynx,
head and neck. This includes diseases of the ear, hearing
loss, allergic and non-allergic diseases of the nose and sinuses, tonsil and adenoid diseases, obstructive sleep
apnea, snoring, voice disorders, and tumors of the head
and neck. Thank you for selecting ent specialists, pc. to
serve your medical needs. We will endeavor to provide
you the very best of care for you and your family. Please review this pamphlet describing our office policies, and if
you have questions, feel free to ask.

OFFICE POLICIES

New Patients
Before you are seen by the doctor, you will be asked
to fill out a form which asks for personal
information, your insurance policy number, and to
sign an assignment of benefit. The office charge
for your first visit will probably be somewhat
higher than for follow-up visits. Your visit may
include additional charges for services such as
hearing testing, sinus irrigation, X-rays, etc.

Office Hours
Our office hours are from 8:00 A.M. to 5:00
P.M., Monday through Friday. After hours, contact
our answering service at 354-2754.

Appointments
Patients are seen by appointment only. You will
be given the first available appointment. If you
cannot keep an appointment, please call in advance
to let us know so that another patient may come in
at that time.

Choice of Doctor
Please schedule your appointment with the
physician of your choice. Patients should feel free
to choose which physician they would like to see. All
our physicians have access to the office chart.

Telephone Policies
Reports on effectiveness of medications or
treatment, or updates on recovery after surgery
often can be communicated to the physician
through an assistant.
Refills are issued during office hours only, and a
doctor must approve the request for refills.
Please furnish the telephone number of your
pharmacy.

Insurance
As a courtesy to our patients we will file your
insurance. All insurance policies are different. You
are responsible to have your current insurance card
with you, and a referral if needed. Please be sure you
have adequate coverage and have followed your
insurance company guidelines. We accept Medicare
assignment and will file with your supplemental
insurance company. Our office will help you with
pre-certification. If you change insurance carriers,
please notify the receptionist.

Fees
Patients co-payments, co-insurance, and
deductibles
are due and payable at time of visit.

OTHER SERVICES

Audiologic Services
We are proud to employ a full-time board
certified Audiologist. We provide comprehensive
audiologic services including hearing tests in
adults and children. We Also provide VNG/ENG testing for balance disorders

Hearing Aid Evaluation & Hearing Aids
Our board certified audiologist, Kenneth Stallons
MS-FAAA, is available for hearing aid evaluations
and consultations. We repair and dispense several different
types and brands of hearing aids. We offer real
ear measurement, visible speech and video otoscopy.

Out-State Clinics
Our physicians operate clinics in nearby
communities. These include: Clarinda, Iowa (biweekly);
and Norfolk (biweekly). Services provided in these
clinics are limited. If you are unsure of where you
should be seen, please call the office or ask your
physician.
